We are now able to issue a birth certificate for any birth record in the state of Ohio. You may request a certified copy of a birth record for anyone born in Ohio. Just complete the request form to place this order.

Births and deaths that occurred within the city limits of Canton from October 1908 through present are registered, filmed and stored. The Division prepares and issues certified copies of birth and death records for a fee of $25.00 per copy.

A certified certificate is a copy of the original record and is required for any legal matter including school registration, drivers license, passport, adoption, retirement benefits, etc.

Corrective affidavits for birth and death records, as well as declaration of paternity forms, are prepared at no charge. For information regarding genealogy requests, please visit the Genealogy Requests page.

WALK IN SERVICE

Birth or death certificates may be purchased Monday – Friday from 8:00 am – 4:30 pm on a walk-in basis at 420 North Market Ave. No identification is required. Payment can be by personal check, cash, VISA or MasterCard.

 

MAIL IN SERVICE

Download one of the forms below to request a copy of a birth certificate or death certificate. The forms are a PDF form that can be filled out and printed on your computer. The mailing instructions are listed on the form. Most requests by mail are answered within 1-2 business days after receipt of the written request. Payment must be personal check or money order.
