Starting on July 1, 2020, there will be a fee to use your credit/debit card at the Health Department. 

  • DEBIT CARDS - Flat fee of $2.00.
  • CREDIT CARDS - $2.00 or 3% of the total amount (whichever is higher)

Canton City Public Health has records for births that occurred in the State of Ohio and for deaths that occurred within the City of Canton limits only.  The cost of each certificate is $25.00 per copy. 

Corrective affidavits for birth records as well as declaration of paternity forms, are prepared at no charge.

Walk In
The Health Department is open Monday through Friday, 8:00am to 4:30pm for walk ins.  We take debit cards, check, cash, money orders and credit cards.  Please follow the below guidelines when coming into the Vital Statistics lobby.

  • Maintain six-foot physical distancing in the lobby.
  • Only four (4) people in the lobby at one time.  You will be asked to wait outside in front of the building if there is already 4 people in the lobby.  If you see there is 4 people already in the lobby, please wait outside.
  • Please try to bring only yourself when coming into the office.  Since there may only be 4 people at a time in the lobby, it is best that you do not bring children or others in with you.
  • Wear a face covering (unless medically unable).
  • Complete the health department's birth/death certificate application prior to arriving (if you can).
  • Bring your own pen (if needed).
  • If you are sick, please stay home.
  • Check your temperature before coming into the health department.  If your temperature is 100 degrees or higher, do not come into the office.
  • There will be a process where you wait in line until you are called to the counter.
  • If you plan to be here for a longer period of time, one of our clerks may ask you to wait in your care or outside.

Mail In Service
Click here for the Birth/Death Certificate Request Form.  Mailing instructions are listed on the form.  Most requests by mail are answered within two business days after the receipt of the written request.  Payments must be a personal check or money order.  Please include a telephone number on all checks. All orders outside of the U.S. must be mailed in.  

Online Ordering
Click here for the Certificate On-Line Ordering.  Your online order will be processed either the same day it was received or the next business day.  Please be aware that there will be a charge involved for paying online. 


NOTEWhen birth or death certificates are mailed out, they will be folded to fit in a regular sized envelope.
