Genealogy Requests

Genealogy requests are taken at the Vital Statistics counter Monday through Friday from 8:15 am to 10:00 am and from 2:00 pm to 4:00 pm.  Up to five (5) genealogy requests per family per day will be completed.  Additional requests may be left at the Vital Statistics counter and will be completed within one to three business days.  E-mail and telephone requests cannot be accommodated.

A genealogy request form must be completed for each request.

The Health Department Vital Statistics office can help you obtain information off of a death or birth certificate.  We cannot supply you with an un-certified copy of a death or birth certificate.  If you would like a copy of a birth or death certificate, we can only issue certified copies (these are $25.00 each - http://www.cantonhealth.org/vital/).
