Communicable Disease Statistics (Monthly Board Report)

Births and deaths from October 1908 until recent time are registered, filmed and stored. The Division prepares and issues certified copies of birth and death records for a fee of $18.00 per copy.

Most requests by mail are answered within 1-2 business days after receipt by this Division.

A certified certificate is a copy of the original record and is required for any legal matter including passport, adoption, social security benefits, etc.

Corrective affidavits for birth and death records, as well as declaration of paternity forms, are prepared at no charge.

There is no charge for certified copies issued for the Veterans' Administration, and verification of births and deaths are done for various other government agencies at no cost.

Burial permits are issued to funeral directors.
