H1N1 Clinic Schedule

Second doses of H1N1 are required for all children less than 10 years of age with a minimum of 21 days since the first dose.

H1N1 Vaccinations will be offered at the Canton City Health Department during the following walk-in immunization clinics: 

• Monday and Thursday from 8:00 am to 11:00 am
• 2nd Thursday of every month from 4:00 pm to 6:45 pm

An H1N1 Vaccine Administration Record must be completed for EACH person that will be vaccinated.  This form may be completed at the office prior to your appointment, or by clicking here and bringing the completed form to your appointment.
