Please note: there is a $20.00 administration fee for flu vaccine this year - we will accept cash, check: made out to "FLUSTATIONS" or Visa/MasterCard.
If you have one of the following medical coverages: Medicare Part B, Medicare Railroad, Medicaid, Caresource, United Health Care, Aetna, Aetna Medicare, Cigna, or The Health Plan we will bill your insurance. You must have your insurance card and driver's license with you or you will be charged $20 for vaccination.
We DO NOT accept Buckeye, Unison, Primetime Medicare, or Humana Medicare at this time
