PERSONNEL CLEANLINESS - Personal appearance and hygiene of staff was good. |

  - PREVENTION OF CONTAMINATION FROM HANDS - Observed no bare hand contact of ready-to-eat foods. Gloves and |
  utensils are provided to handle ready to eat foods. |
|
  - PERSON IN CHARGE- Person in Charge demonstrated basic food safety knowledge except as specified in this report. The |
  PIC also could not correctly answer the minimum temperature requirement for reheating TCS foods. Education was provided. Ohio |
  Food Code requirements regarding approved Level 1 and Level 2 Certification in Food Protection was discussed. |

- THAWING - Observed foods thawing in accordance with Ohio Administrative Code. |
|
  - All cold TCS foods were cold holding at 41°F or below except for packaged cheese in display cooler in the market which was |
  holding at 45°F. The PIC was notified of the issue and the cooler thermostat was adjusted. The cooler was cold holding at 37°F |
  by the end of the inspection. All hot TCS foods were hot holding at 135°F or above. Discussed proper reheating with the PIC. |
  TCS foods are reheated for hot holding within 2 hours or less. |

  - DATE MARKING - Observed TCS and ready to eat foods that were held for more than 24 hours and were not datemarked, were |
  improperly datemarked or were over 7 days past the date marked on the container. The issue was discussed with the PIC and |
  education was provided. It was noted that smoked chicken that is smoked on-site must be treated as a TCS food unless a variance |
  is obtained from the Ohio Department of Health for their smoking process to render the smoked chicken a non-tcs food. |

  - CONSUMER ADVISORY- A consumer advisory was provided on the menu. However, no asterisk was provided next to each item |
  that can be ordered raw or undercooked and next to the advisory. The issue was discussed and will be corrected. |
|
  - PROTECTION FROM CONTAMINATION - Observed raw animal foods properly stored and separated from ready-to-eat foods |
  and foods with a lower minimum cooking temperature. Observed some residue buildup on clean food equipment and utensils. The issue |
  was discussed and will be corrected. |
