  1) Good Hygienic Practices     Food employees personal hygiene   and appearance are good. No discharge from eyes, nose, and
  mouth   noted.
  
  
  2)  Preventing Contamination by Hands   - Handwashing  facilities are properly supplied and accessible. (Observed) No bare
  hand  contact with ready-to-eat foods. Gloves, tongs, and spoons used  while handling ready -to- eat food.
  
  
  VI.  Time/Temperature  Controlled Safety Food  -  Hot food   holding temperatures and cold food holding temperatures are  
  good.; Date marking noted on prepared food and opened food items. Including day of preparation as day 1, a maximum of 7 days is 
  allowed, afterwhich the product must be discarded. Reheating to  165° as required within 2 hrs.
  
  
  VII  Protection from Contamination   - Observed raw animal meats  properly separated by types and properly protected from 
  contamination with ready-to-eat and raw ready-to eat foods.
  
  
  I. Employee Health  - Person in charge is not aware of Ohio  Administrative Code requirements, and has no employee health 
  policy or employee health agreement form. I discussed this the  employee form agreement form and policy at this time with the 
  owner / operator. I gave him other handouts.about vomit/ diarrhea and kit for vomit /diarrhea, allergen and discussed them.
