II - PERSONNEL CLEANLINESS - Personal appearance and hygiene of staff was good. |
  III - PREVENTION OF CONTAMINATION FROM HANDS - Observed no bare hand contact of ready-to-eat foods. However, |
  it was noted that neither handwash sink had paper towels. The chef was using a cloth towel to dry hands. The issue was discussed |
  with the chef and Person in Charge and will be corrected. |
IV - PERSON IN CHARGE- Person in Charge is Serv Safe certified. |
  VII - Observed proper cold holding temperatures as specified in Ohio Administrative Code. All potentially hazardous foods |
  that have been cooked are refrigerated and reheated for hot holding within 2 hours to 165F; are cold held below 41F; and hot held |
  above 135F. Cooked foods are cooled to 70F within 2 hours and from 70F to 41F or below within 4 hours. |
  VIII - DATE MARKING - Observed some unpackaged potentially hazardous and ready to eat foods that were expired and |
  prociutto that was held for more than 24 hours and was not datemarked. The issue was discussed with the Person in Charge and will |
  be corrected. Discard foods as they expire and properly date mark foods as required per rule requirements. |
IX - CONSUMER ADVISORY - A consumer advisory was provided on the menu. |
  XI - PROTECTION FROM CONTAMINATION - Observed raw animal foods properly stored and separated from ready-to-eat |
  foods except for raw hamburger stored above raw whole muscle beef steaks in walk in cooler on cart and raw chicken stored above |
  raw whole muscle beef steaks in prep cooler. The issue was discussed with the Person in Charge and was corrected immediately. |
  Properly store and separate raw animal foods from foods with a lower minimum cooking temperature. |
  XIV- TRANSPORTING FOOD OFF PREMISES - This facility is not currently catering off site until the appropriate |
  equipment is acquired. |
XV- TEMPERATURE MEASURING DEVICE - Temperature measuring devices were provided. |
