  - Employee Health - The PIC did not have sufficient documentation that employees are informed of their responsibility to |
  report information about their health as it relates to foodborne diseases to the PIC. She was also unaware that there are 13 |
  reportable foodborne diseases and not 6 as stated on their employee health reporting poster. The issue was discussed with the |
  PIC. |

- Personnel Cleanliness - Personal appearance and hygiene of staff was good. |
|
  - Prevention of Contamination from Hands - Observed no bare hand contact of ready-to-eat foods. Observed good handwashing and |
  glove use. |
|
  - Person In Charge - Person in Charge demonstrated basic food safety knowledge and is Serv Safe Certified. Application has |
  been made to obtain ODH Level 2 Certification in Food Protection. |
|
- Thawing - Foods are thawed in accordance with the Ohio Administrative Code. |
|
  - Observed proper cooking temperatures and parameters as specified in Ohio Administrative Code. This facility also has an |
  approved time in lieu of temperature policy. TCS foods under the TILT policy were properly time marked. At the time of |
  inspection, all hot foods were hot holding above 135°F and all coolers were cold holding below 41°F. |

- Date Marking - Observed good date marking. |
|
  - Protection from Contamination - Observed raw animal foods properly stored and separated from ready-to-eat foods and foods with |
  a lower minimum cooking temperature. Observed a buildup of residue on clean food equipment, utensils and on dispensers at fountain |
  beverage machines. The issue was discussed with the Person in Charge and will be corrected. Also, observed a spray bottle of |
  bathroom cleaner stored above packages of ground coffee and coffee filters. Properly store toxic chemicals in a manner that |
  prevent chemical contamination of food, food equipment and single use / single service items. The issue was discussed and |
  corrected immediately. |
