1) Personal Cleanliness - Food handlers personal hygiene and cleanliness are good. |

  2) Hand washing, Prevention of Contamination from Hands - Hand washing facilities are adequate for employees. Hand washing |
  facilities are properly supplied. Exposed ready-to-eat foods are not touched by bare hands. Gloves, tongs are used while handling |
  ready to eat food. |

  3) Cooking, Reheating, Cooling, Hot and Cold Holding - Proper cooking temperatures and parameters are as specified in Ohio |
  Administrative Code. Hot foods are being held at 135°F or greater. Observed cold foods being held at 42-45°F under |
  refrigeration including but not limited to egg and sausage mix, ham and eggs. Cold food must be held at 41°F or below. I |
  discussed this issue with the manager she stated that the cooler temperature was at 39°F this morning. These food products were |
  transferred to another cooler holding at 41°F or below. The manager agreed not to use the prep cooler until it has been repaired |
  to cold hold at 41°F or below at all times. |

  3) Date Marking/time as a Public Health Control - Potentially hazardous ready to eat food left out at room temperature is |
  under an approved Time in Lieu of Temperature policy. Observed time stamp on each food product stored this way. Time used as a |
  public health control. The food is served within 4 hours or less. It was noted that the processed american cheese was not time |
  stamped according to the written policy. This facility has a variance to hold the processed american cheese at room temperature |
  for 12 hours. The issue was discussed with the manager and was corrected immediately. |

  4) Protection from Contamination - Raw animal foods are separated from raw ready to eat foods and cooked ready to eat foods |
  except for raw shell eggs in prep cooler stored above fully cooked fold over eggs. The issue was discussed with the manager and |
  will be corrected. |

5) Temperature Measuring Devices - Temperature measuring devices are provided and accessible. |
|
Note: All coolers were cold holding at 41°F or below except prep cooler cited above. |
