  Comments:
  1) Hand washing (Observed) Hand washing facilities adequate. Exposed ready-to-eat foods not touched by bare hands. Cooks and
  kitchen staff use vinyl gloves, tongs, spoons, spatulas while handling ready to eat foods.
  
  2) Person in charge demonstrates knowledge with responses to food safety questions regarding the operation. Person(s)
  responsible for the operations of this facility has taken the Level 2 Servsafe certification course, but has not been certified
  by ODH.
  
  3) Potentially hazardous foods that have been cooked and  refrigerated are reheated for hot holding within 2 hours to  165°F.
  Cooked potentially hazardous foods are cooled from 135°F  to 70°F within 2 hours and from 70°F to 41°F or below within 4 hours.
  Soups are chilled with the use of chill sticks. Hot foods held at 135°F or greater, cold  foods held at 41°F or below.
  Time/temperature controlled for safety food date marked. (Many prepared foods and opened foods are used within a 24 hr period.)
  
  5) Consumers informed about increased risk when eating animal foods in a raw or undercooked form.
  
  6) Raw animal foods separated from raw ready to eat foods / cooked ready to eat foods.
