  AND WILL BE CORRECTED. ALL PRE-PACKAGED FOODS OFFERED FOR CONSUMER SELF SERVICE MUST BE
  PROPERLY LABELED PER RULE REQUIREMENTS.

  (A) Standards of identify.  Packaged food shall comply with  standard of identity requirements in 21 C.F.R. 131-169 and
  9 C.F.R. 319, and the general requirements in 21 C.F.R. 130 and 9 C.F.R. 319 Subpart A.

  TO THE WALL. RECAULK BEHIND HANDWASH SINKS SO THAT THEY ARE PROPERLY SEALED TO THE WALL PER
  RULE REQUIREMENTS.

  CLEAN UP MOUSE DROPPINGS AS OFTEN AS NECESSARY TO MAINTAIN EQUIPMENT CLEAN.

  OBSERVED BOTH EXTERIOR DOORS PROPPED OPEN. EXTERIOR DOORS MUST BE KEPT CLOSED TO PREVENT THE
  ENTRANCE OF RODENTS, INSECTS AND OTHER PESTS. THE ISSUE WAS DISCUSSED WITH THE PERSON IN
  CHARGE AND WILL BE CORRECTED.

  NECESSARY TO MAINTAIN CLEAN. ALSO, OBSERVED EXCESS MATERIALS AND CLUTTER STORED IN STORAGE
  ROOMS AND THROUGHOUT THE FACILITY. REMOVE BROKEN OR UNNECESSARY ITEMS FROM THE FACILITY.
  ORGANIZE STORAGE AREAS TO FACILITATE FLOOR CLEANING AND TO PREVENT OTHER HAZARDS.

  OBSERVED WET MOP STORED IN MOP BUCKET. MOP HOOKS WERE PROVIDED BUT WERE INACCESSIBLE. REMOVE
  CLUTTER AROUND MOP HOOKS SO THAT MOP HOOKS ARE ACCESSIBLE. WET MOPS MUST BE PROPERLY HUNG
  AFTER USE SO THAT THEY CAN AIR DRY PER RULE REQUIREMENTS.

  (F)Drying mops.
  After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment,
  or supplies.

  (H)Cleaning and maintenance of plumbing fixtures.
  Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary
  to keep them clean and maintained.

  AS NOTED ABOVE, MOUSE DROPPINGS WERE OBSERVED INSIDE CABINETS IN AREA FORMERLY USED AS A
  LICENSED KITCHEN. THE ISSUE WAS DISCUSSED WITH THE PERSON IN CHARGE. HE AGREED TO CONTACT A
  LICENSED PEST CONTROL COMPANY TO ADDRESS THE ISSUE. CLEAN UP DROPPINGS REGULARLY TO MAINTAIN
  THE FACILITY IN SANITARY CONDITION AND TO MONITOR CURRENT RODENT ACTIVITY. KEEP EXTERIOR DOORS
  CLOSED TO PREVENT THE ENTRANCE OF RODENTS, INSECTS AND OTHER PESTS. THE PRESENCE OF RODENTS,
  INSECTS AND OTHER PESTS ON THE PREMISES MUST BE MINIMIZED. (CRITICAL VIOLATION)

  (K)Controlling pests.
  The presence of insects, rodents, and other pests shall be  controlled to minimize their presence on the premises
  by:
      (1) Routinely inspecting incoming shipments of food and  supplies;
      (2) Routinely inspecting the premises for evidence of pests;
      (3) Using methods, if pests are found, such as trapping  devices or other means of pest control as specified
      under paragraphs (C), (K) and (L) of rule 3717-1-07.1 of the  Administrative Code; and
      (4) Eliminating harborage conditions.

  The premises shall be free of:
      (1) Items that are unnecessary to the operation or  maintenance of the food service operation or retail food
      establishment such as equipment that is nonfunctional or no  longer used; and
      (2) Litter.

  OBSERVED TOXIC CHEMICALS STORED ABOVE BABY BOTTLE LINERS ON RETAIL SHELVES. PROPERLY STORE AND
  SEPARATE TOXIC CHEMICALS FROM SINGLE SERVICE / SINGLE USE ITEMS TO PREVENT CHEMICAL
  CONTAMINATION. THE ISSUE WAS DISCUSSED AND CORRECTED IMMEDIATELY. (CRITICAL VIOLATION)

  (A)Storage: separation.

  ALL COOLERS WERE COLD HOLDING AT 41°F OR BELOW.
   
  NOTES:
   
  1.) OBSERVED A TODDLER BED IN BACK ROOM. IT WAS NOTED THAT LIVING QUARTERS CAN NOT BE LOCATED
  IN AREAS WHERE RETAIL FOOD ESTABLISHMENT ACTIVITIES TAKE PLACE. THE BED SHOULD BE REMOVED
  FROM THE PREMISES. THE PERSON IN CHARGE STATED THAT IT WAS THERE FROM A FORMER EMPLOYEE WHO
  BROUGHT HER YOUNG CHILDREN TO WORK WITH HER.
   
  2.) IT WAS RECOMMENDED THAT THE METAL GATE IN FRONT OF BACK EXIT  / DELIVERY DOOR BE REMOVED.
   
  3.) EMPLOYEE REFRIGERATED AND FROZEN FOODS SHOULD BE STORED SEPARATE FROM FOODS OFFERED FOR
  SALE IN CLEARLY DESIGNATED COOLERS / FREEZERS. IT IS RECOMMENDED THAT THESE COOLERS AND
  FREEZERS BE LOCATED IN AN AREA THAT IS INACCESSIBLE TO THE PUBLIC.
   
  ***A FOLLOW UP INSPECTION WILL CONDUCTED ON MONDAY, JULY 11, 2016, AT WHICH TIME ALL
  VIOLATIONS MUST BE CORRECTED.***
