  THIS WAS A CONSULTATION REGARDING THE PROPOSED NEW LOCATION AT 909 12TH ST NW. THE OPERATOR WAS
  INFORMED THAT THEY WILL HAVE TO SUBMIT PLANS WITH A PLAN REVIEW AND APPROPRIATE FEE FOR A
  REMODEL. THE OPERATOR WAS INFORMED THAT THEY WILL NEED TO INSTALL A HANDWASH SINK THAT MEETS
  ALL BUILDING AND CODE REQUIREMENTS AS WELL AS A UTILITY / MOP SINK. ALSO, A TERMITE / CARPENTER
  ANT INFESTATION WAS OBSERVED DURING THIS CONSULTATION. THE OPERATOR WAS INFORMED THAT A
  LICENSED PEST CONTROL COMPANY WOULD NEED TO COME AND ABATE THE PEST PROBLEM. THE FACILITY WILL
  HAVE TO MEET ALL BUILDING AND CODE REQUIREMENTS AND RETAIL FOOD ESTABLISHMENT REQUIREMENTS
  SPECIFIED UNDER THE OHIO ADMINISTRATIVE CODE BEFORE THE FACILITY CAN BE LICENSED.
