  Violation: Boxes of chips are stored on the floor in what used  to be the store.
   
  3717-1-03.2 Food: protection from contamination after receiving.
  (Q)Food storage - preventing contamination from the premises.
  Except as hereinafter provided in this rule, food shall be  protected from contamination by storing the food:
      (1) In a clean, dry location;
      (2) Where it is not exposed to splash, dust, or other  contamination; and
      (3) At least six inches (fifteen centimeters) above the  floor.
  Food in packages and working containers may be stored less than  six inches (fifteen centimeters) above the floor
  on case lot handling equipment as specified under paragraph (II)  of rule 3717-1-04.1 of the Administrative Code; and
  pressurized beverage containers, cased food in waterproof  containers such as bottles or cans, and milk containers in
  plastic crates may be stored on a floor that is clean and not  exposed to floor moisture.

  Violation: Observed sugar stored on a shelf beside cleaners.
   
  3717-1-07.2 Poisonous or toxic materials: storage and display -  separation.
  Poisonous or toxic materials shall be stored and displayed for  retail sale so they can not contaminate food,
  equipment, utensils, linens, single-service articles, or  single-use articles by:
  (A)Separating the poisonous or toxic materials by spacing or  partitioning; and

  (A)Storage: separation.
  Poisonous or toxic materials shall be stored so they can not  contaminate food, equipment, utensils, linens,
  single-service articles, and single-use articles by:
      (1) Separating the poisonous or toxic materials by spacing or partitioning; and
      (2) Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens,
      single-service articles, or single-use articles. This  paragraph does not apply to equipment and utensil cleaners
      and sanitizers that are stored in warewashing areas for  availability and convenience if the materials are stored
      to prevent contamination of food, equipment, utensils,  linens, single-service articles, and single-use articles.

  Violation: There is some kind of oily residue on and under the Budweiser reach in cooler. Many coolers in the facility are
  non-functioning.

  (A)Repairing.
  The physical facilities shall be maintained in good repair.

  3717-1-06.4 Physical facilities: maintenance and operation.
  (N)Maintaining premises - unnecessary items and litter.
  The premises shall be free of:
      (1) Items that are unnecessary to the operation or  maintenance of the food service operation or retail food
      establishment such as equipment that is nonfunctional or no  longer used; and
      (2) Litter.

  Violation: Handsinks in the bathrooms do not have handwashing  signs. These were provided to the operator.
   
  3717-1-06.2 Physical facilities: numbers and capacities.
  (E)Handwashing signage.
  A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used
  by food employees and shall be clearly visible to food employees.

  Violation: There is not a waste receptacle in the bathrooms. There needs to be one to dispose of used paper towels after
  washing, but also a covered receptacle in the restroom for feminine supplies.
   
  3717-1-06.2 Physical facilities: numbers and capacities.
  (F)Disposable towels - waste receptacle.
  A handwashing sink or group of adjacent handwashing sinks that is provided with disposable towels shall be provided
  with a waste receptacle as specified under paragraph (G)    (3)  of rule 3717-1-05.4 of the Administrative Code.

  Violation: Multiple coolers have unshielded light bulbs within them. The bulbs over stored items in the drive thru and
  storage areas are also not shielded.
   
  3717-1-06.1 Physical facilities: design, construction, and  installation.
  (I)Light bulbs - protective shielding.
      (1) Light bulbs shall be shielded, coated, or otherwise  shatter-resistant in areas where there is exposed food;
      clean equipment, utensils, or linens; or unwrapped  single-service or single-use articles. This paragraph does
      not apply in areas used only for storing food in unopened  packages, if the integrity of the packages can not be
      affected by broken glass falling onto them; and the packages  are capable of being cleaned of debris from broken
      bulbs before the packages are opened.
      (2) An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending
      beyond the bulb so that only the face of the bulb is exposed.

  Cooler temperatures are 41°F or less for all coolers. There is  little TCS food being sold here.
  This facility is only selling pre-packaged items and will be a Class 1 facility next year. The bar and convenience store are
  no longer open to the public.
