  Since the last inspection, the previously documented violations have been addressed. The walk-in cooler was holding food at
  37F. They have bleach and quat tablets available at the facility and test strips for each. Broken plastic containers have
  been discarded. Cleaning has been completed. All lights have been repaired.
   
  The GM has an ODH level II food safety certificate. The owner has level I food safety certificate. He plans on training
  more staff to ensure there is always a level I or level II certified person at the store at all times.
   
  Employee health training is not verifiable at this time.  I'll email a sample form with inspection report.
