  The physical facilities shall be maintained in good repair.

  VIOLATION 1) The toilet bowl in the restroom is not clean and heavily stained. Clean, sanitize, and maintain toilet plumbing
   fixture, or replace the toilet fixture

  (H)Cleaning and maintenance of plumbing fixtures.
  Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary
  to keep them clean and maintained.

  VIOLATION - 1) The door to the walk-in cooler has been repaired, but the frame around the door is bare wood. Refinish the
  bare wood and make smooth cleanable surface.2) Some work has been done in the drive thru walk-in cooler.The wall surfaces
  (drive thru walk-in cooler) are still torn, worn ,and not cleanable in several areas. Repair the wall surfaces in the
  walk-in cooler (drive thru). 3) The store walk-in cooler walls, ceiling, and shelving worn are,worn, discolored and not
  easily cleanable. Refinish / repaint all surfaces in the walk-in cooler.
   
  3717-1-04.4 Equipment, utensils, and linens: maintenance and operation
  (A)Equipment - good repair and proper adjustment.
      (1) Equipment shall be maintained in a state of repair and condition that meets the requirements specified
      under rule 3717-1-04 of the Administrative Code and rule 3717-1-04.1 of the Administrative Code.
      (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight,
      and adjusted in accordance with manufacturer's specifications.
      (3) Cutting or piercing parts of can openers shall be kept sharp to minimize the creation of metal fragments
      that can contaminate food when the container is opened.

  VIOLATION 1) The restroom handwashig sink and handsink in work area are not sealed to the wall. Seal these handwashing sinks
  that are fixed to the wall.

      (1) Equipment that is fixed because it is not easily movable shall be installed so that it is:
                  (c) Sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage.
