Information needed for WIC appointments

In order to determine WIC eligibility, please bring the following to your appointment:

1. Proof of Income (THREE most recent pay stubs AND current medical card, if received)
2. If pregnant, a doctor's statement showing DUE DATE
3. Identification
       Child Identification
           ► I.D. with child's birthday listed on it - Birth Certificate OR Crib Card
       Adult Identification
           ► Birth Certificate
           ► Driver's License
           ► Social Security Card
           ► Health Card/Insurance Card
           ► Employee ID Card
4. Child (Only if the appointment relates to the child)
5. Child's shot records (Only if appointment relates to the child)
6. Proof of address (name and address)
